1. Download the Application Kit (files are in *PDF format):

2. Fill up the application form. Please read and follow the guidelines for the application procedures;
   
   
3. After filling up the application form, send (email/fax/mail) it to CITEM including all other required documents and deposit slip of reservation fee to the following:
